Patient Education & FAQs
- Patients need to bring a picture ID and current insurance card to the appointment.
- We recommend patients arrive a minimum of 15 minutes before their appointment times to complete the check in process.
- For HMO patients, we ask that you verify with your insurance company that our physicians have been registered as the patient’s PCPs before arrival. If we are not listed as such, you (the patient) are considered self pay for that visit.
- Medicare Advantage Plan patients, please bring all specialists’ names, addresses, future appointment dates and records available to your first appointment to ensure a smooth referral process.
